The City Clerk’s Office is responsible for original records of all deaths that have occurred in Gardner, including Heywood Hospital. All death certificates are public records. A certified copy of a death certificate may be obtained in person or by mail.
Please note, records requested for Genealogical Research may take up to 10 business days to process.
Fee
The fee is $10 per copy.
How to Obtain a Death Certificate
- In person: Walk-in requests are accepted and you may wait while the certificates are prepared. Cash and checks are accepted forms of payment.
- By mail: Please fill out the attached Death Certificate Request Form (PDF) and return with a check or money order payable to the "City of Gardner.
